Each dungeon has 3 areas connected through one-way-portals, each with a boss. There's always a main boss and 2 (random) sidebosses. Dungeons also come in 3 levels of difficulty: Normal, Nightmare and Hell. To unlock the higher difficulties you must have completed the previous level, each difficulty is a 'new' dungeon, thus needs a dungeon key again.
The monster and boss strength increases greatly with each difficulty, the resources also increase. Base amount is 10,000. Each dungeon tier adds 5k, each difficulty ads the base sum. So for resources it is more useful to do, e.g., d4hell (20k * 3 = 60k) than d5nightmare (25k * 2 = 50k). Although the higher tier dungeons would still give higher leveled loot.
Question marks are (randomly) monsters, chests or resources.
There is a chance that each dungeon will only spawn 2 of the 3 potential bosses with each key use. You will still have to fight 3 bosses (mini & final) but 2 of them maybe the same boss.
